My plan is to pull the plugs and take a look. I have a bunch of old spark plugs that I can re-gap and throw back in. If one plug is indeed fouled, I will switch the injector from that cylinder with another cylinder, to see if the foul follows the injector.

I think this is a minor issue unless the bores got washed out real bad. I pulled the #2/4 plugs... #4 was wet. I swapped the coil packs between cylinders, put the plug from #2 into #4, and put a stock heat range ngk iridium into #2. It ran fine after starting up but did the same thing by the time the oil came up to temp (zero boost too). What I do next depends on which plug is wet. I have a feeling #2 will be.

Taking a break putting things back together. The problem stayed in #4. The #4 injector clip was not clicked in. That one is a real bish to get at and I missed it earlier when I checked the other ones. I did a visual and it looked ok, but it pulled right off. The plugs on the passenger side looked fine. Maybe this explains why the idle fuel trims were off too.

I drove around for 20 minutes and did a bunch of pulls at ~15psi and everything seems good after plugging that injector back in.
